(a) Doctors Visits

(i) General Practitioner

·         Office Visit

$47.00 per visit (8.00 am. To 6.00 pm.)

·         Visit by doctor to site

$94.00 per visit (8.00 am. To 6.00 pm.)

·         Emergency visit

N/A

(ii) Specialist visit

·         Office Visit

$117.00 per visit (8.00 am. To 6.00 pm.)

·         Visit by doctor to site

$155.00 per visit (8.00 am. To 6.00 pm.)

(iii) Psychiatrist

·         Initial consultation

$140.00 per hour

·         Follow up

$117.00 per visit to maximum of 15 visits

(b) Drugs and Dressing

$775.00 per injury

(c) Hospital Expenses

$233.00 per day including cost of investigation, drugs and x-rays

(d) Operations

l Minor

Up to $620.00

l Intermediate

Up to $1,240.00

l Major

Up to $2,480.00

(e) Magnetic Resonance Imaging

Up to $2,000.00 for each body part

Maximum Liability:

 $22,500.00
